A participant is connected with an oncology nurse who can:

  • Be available to call and help address your questions or concerns.
  • Help you understand your options and provide information to decide on a treatment plan.
  • Coordinate your care with your oncology team and Centers of Excellence facility, if you access one for your care.
  • Help you understand what to expect before, during and after treatment.

Every year, each facility in the network is evaluated to ensure it meets high quality standards to qualify as a “Center of Excellence.” The COE network includes some of the top facilities in the United States, which feature:

  • Access to newer treatments that may be the best option for you.
  • Second opinion services if you are unsure about your diagnosis or what treatment may be right for you.
  • Experience in performing a large number of cancer surgeries and other complex treatments and procedures.
  • Comprehensive, highly specialized teams with extensive experience in cancer diagnosis and treatment, including rare cancers.
  • Your nurse will ask you to fill out a Release of Information form, which will allow the nurse to help coordinate your care.
  • If a nurse needs to discuss your health information with an individual that you designate or get health information from a third party, then you’ll need to complete and sign an Authorization for Release of Information form.
  • The Release of Information form gives your nurse permission to discuss personal health information with another party in order to better coordinate care.
  • Note: It is a federal law that a nurse is not allowed to discuss personal health information with your spouse or other covered dependent without your consent.
